Site Specific Items

Sign Safety Meetings / Permits / Site Paperwork

Depending on your site you may need to sign safety meetings, work permits, or additional items. Ensure you ask dispatch or your consultant if there is any specifics they need from you

Medical Billing Form

Please ensure you are filling out the medical billing form to pass along any information on your site. This is completed at the beginning of your rotation, anytime your site changes, and any crew changes occur.
